Does CHAMPVA cover surgery? (Including cataract and weight-loss surgery)
Educational info, not medical advice. Last verified Oct 11, 2026.
Coverage status: Covered with conditions
Short answer: Yes, CHAMPVA covers medically necessary surgery, but cosmetic surgery isn’t covered, organ transplants need approval first, and weight-loss surgery has strict BMI rules.
Key conditions
CHAMPVA covers care that is medically necessary and appropriate (38 CFR 17.272). The Guidebook doesn’t have a single “surgery” section; rules for specific surgeries appear throughout it.
- Cosmetic surgery (mainly to improve appearance) is not covered.
- Reconstructive surgery is covered in listed cases, such as breast reconstruction after a medically necessary mastectomy or for a congenital anomaly, cleft palate repair, and tongue-tie surgery.
- Organ transplants need preauthorization (38 CFR 17.273).
Weight-loss (bariatric) surgery
Covered when BMI is over 40, or over 35 with serious medical conditions caused or worsened by obesity, or for a second surgery to fix complications. Covered procedures: adjustable gastric banding, gastroplasty, Roux-en-Y gastric bypass, and vertical banded gastroplasty. The claim must include BMI, height, weight, medical history, and other treatments tried. Weight-loss drugs: see GLP-1 coverage.
Eye surgery
- Eyelid surgery (blepharoplasty): covered when it fixes a significant vision problem, with two visual field studies and photos.
- Cataract surgery: the Guidebook doesn’t address cataract surgery by name. The federal rule allows lenses when needed to replace the eye’s lens lost to eye surgery or injury (38 CFR 17.272). Call CHAMPVA to confirm before you schedule.
What you pay
The Guidebook’s cost summary: ambulatory surgery: no deductible, 25% of the allowable amount. Surgeon and other professional fees: deductible plus 25%. For a hospital stay there is no deductible; you pay the inpatient cost share in the Guidebook (the lesser of a per-day amount times the days, 25% of the billed amount, or the base DRG rate at DRG-based hospitals). When CHAMPVA is secondary, you usually pay nothing.
Not sure? Call CHAMPVA at 800-733-8387 (TTY 711), Monday–Friday, 8:00 a.m.–7:30 p.m. ET. The Guidebook says a service listed as covered doesn't guarantee payment. See all CHAMPVA coverage answers.
