Does CHAMPVA cover physical therapy, speech therapy, or massage therapy?
Educational info, not medical advice. Last verified Oct 11, 2026.
Coverage status: Covered with conditions
Short answer: Physical, occupational, and speech therapy are covered when prescribed and medically necessary; the Guidebook doesn’t address massage therapy, and chiropractic care and acupuncture are not covered.
Key conditions
- Physical therapy: prescribed by a physician, physician assistant, or certified nurse practitioner; covered when it improves muscle strength, joint motion, coordination, and endurance.
- Occupational therapy: prescribed, medically necessary, and expected to improve, restore, or maintain function in a reasonable time.
- Speech therapy: for physical impairments such as brain injury or stroke, cleft lip or palate, cerebral palsy and other neuromuscular disorders, and congenital sensory disorders. For children 3–21, schools must provide speech therapy under IDEA; if the state doesn’t, documentation from the state is required.
- Massage therapy: the Guidebook doesn’t address it. Call CHAMPVA before you start.
- Not covered: chiropractic care, acupuncture, health club memberships, exercise equipment. See Chiropractic.
Preauthorization
The Guidebook doesn’t list these therapies as needing preauthorization. CHAMPVA may review long-running therapy and ask for medical records or a treatment plan.
What you pay
When CHAMPVA is primary, you pay the yearly outpatient deductible ($50 per person, $100 per family) and a 25% cost share. When CHAMPVA is secondary, you usually pay nothing. Your family’s costs are capped at $3,000 per calendar year (Guidebook).
Not sure? Call CHAMPVA at 800-733-8387 (TTY 711), Monday–Friday, 8:00 a.m.–7:30 p.m. ET. The Guidebook says a service listed as covered doesn't guarantee payment. See all CHAMPVA coverage answers.
