What you pay depends on whether you’re admitted
The CHAMPVA Guidebook’s cost summary (no other health insurance) says:

Your family’s cost share is capped at $3,000 per calendar year for covered care. After that, CHAMPVA pays 100% of the allowable amount for the rest of the year.
Ambulance
Ambulance service is covered when life-sustaining equipment is needed for a covered condition. Air ambulance to the nearest facility that can treat you is covered if ground transport isn’t available or suitable and your condition needs immediate evacuation. Transportation that doesn’t require life-sustaining equipment isn’t covered. Ambulance is billed as outpatient: deductible plus 25% when CHAMPVA is primary.
There’s no CHAMPVA network
VA says there’s no specific network of CHAMPVA providers. Hospitals that accept Medicare must also accept CHAMPVA. If a provider doesn’t accept CHAMPVA, CHAMPVA may still pay its allowable amount, but you may have to pay the provider and file for reimbursement, and you owe anything above the allowable amount.
Traveling abroad? The Guidebook says CHAMPVA provides the same benefits overseas (except in countries restricted by the U.S. Treasury), with the same deductible and cost share.
Filing the claim
- Most hospitals file for you. If you paid yourself, file VA Form 10-7959a with an itemized bill.
- Deadline: 1 year after the date of service; for an inpatient stay, 1 year after discharge.
