- Enroll in Medicare Part A and Part B (or a Medicare Advantage plan) through Social Security
- Send CHAMPVA: VA Form 10-7959c + front and back of the Medicare card
- Have Part D? Also send the Part D card. (Part D is optional; with it, you can’t use Meds by Mail)
- Not eligible for Medicare? Get a notice of disallowance from Social Security and send it to CHAMPVA
- Remember: CHAMPVA doesn’t pay Part B premiums
